Dear all,
I'm development a clinical protocol for our forensic in-patients. There are
several keys elements in the clinical protocol.

1. Intake assessment (data collection, needs assessment and risk assessment)
2. agreed care plan
3. Further assessment and OT program
4. Community transition program for d/c preparation.

Since concept of agreed care plang is new to me, please advise me how to develop
a agreed care plan for OT intervention. any standarised form or documentation
are used for agreed care plan.
