I'm not sure if it could apply on your situation but using RVRD should able to bridge two multicast network with same subject name

If we're talking disparate networks here -- maybe iptables and nat ( or a twice
nat? ) could do a trick for you ?

How do the heartbeats hit the clients ? broadcast? unicast ? tcp?

We don't run a Hawk over on this side of the globe, perhaps a quick peek with
tcpdump would give a good idea of how things work out. A solution might stand
out from there.

Hi there unfortunately, no matter what way you look at this, you'll need somekind of bridge or gateway to reach any remote network, and from the sounds of it,...
