I'm not sure if it could apply on your situation but using RVRD should able to bridge two multicast network with same subject name

If we're talking disparate networks here -- maybe iptables and nat ( or a twice
nat? ) could do a trick for you ?
How do the heartbeats hit the clients ? broadcast? unicast ? tcp?
We don't run a Hawk over on this side of the globe, perhaps a quick peek with
tcpdump would give a good idea of how things work out. A solution might stand
out from there.
