First download and install Unlocker http://ccollomb.free.fr/unlocker/
Then find one of TinyCads libraries and right mouse click and run unlocker to see if any program has it locked. If so that's your culprit if not then download and install ShellExtViewer http://www.nirsoft.net/utils/shexview.html
Now find the non Microsoft extensions and disable anyone you think might be the culprit and then try to open or create a TinyCAD library, if you can then one of them is the culprit and it's just a matter of enabling one at a time until you find the one.
If you have no idea disable about half of the non-Microsoft ones, reboot your computer and then test, if that isn't it reanable and disable the other half and try again.
If all this still fails, hmmm you would be willing to run HiJackThis http://www.spywareinfo.com/~merijn/programs.php#hijackthis
You can then put the log file here http://www.hijackthis.de/ and see if there are little nasties that might be interfering.
